银屑病俗称牛皮癣,是常见的皮肤慢性炎症性疾病。该病易于复发、无法根治、严重危害患者身心健康。临床上包括anti-TNF、anti-IL-17A/IL-17R、anti-IL-23等在内的抗体类药物靶向免疫系统及相关因子治疗银屑病有良好预期和效果,但副作用大,停药后疾病易反弹。因此,深入研究银屑病的发病机制、治疗新靶点和新策略势在必行。近日,上海交通大学基础医学院王宏林教授团队在国际免疫学顶级期刊Immunity在线发表了题目为“Excessive Polyamine Generation in Keratinocytes Promotes Self-RNA Sensing by Dendritic Cells in Psoriasis”研究论文,该研究发现白细胞介素17 (IL-17)下调了银屑病角质形成细胞中的蛋白磷酸酶6(PP6),导致磷酸化和转录因子C/EBP-β的激活以及随后产生的精氨酸酶-1。角质形成细胞中缺乏PP6的小鼠易患牛皮癣样皮肤炎症。

该研究发现,PP6缺失的表皮细胞精氨酸酶(Arg1)上调导致尿素循环下游代谢物——多胺(Polyamines)异常地在皮肤部位累积。Polyamines携同表皮细胞释放的自身核酸与具有RNA结合能力的短肽形成复合物,而来自表皮细胞的多胺+自身核酸+短肽复合物形成自身抗原进入树突状细胞(DCs)后强烈激活内体中的Toll样受体-7,并介导DCs释放多种促炎因子。皮肤局部注射精氨酸酶抑制剂显著改善小鼠与非人灵长类动物模型的类银屑病皮肤炎症。该研究首次揭示,组织特异性细胞的代谢重编程所引起的代谢物异常累积是银屑病等慢性炎症性疾病发生发展的重要帮凶,表皮细胞的多胺+自身核酸+短肽复合物是激活皮肤免疫反应的自身抗原,而靶向调节表皮细胞精氨酸代谢通路的异常有望成为治疗银屑病的新策略。(文章内容参考https://www.cell.com/immunity/fulltext/S1074-7613(20)30234-X,https://www.shsmu.edu.cn/cbms/info/1077/3856.htm
